While hyperv.rst already has all currently implemented Hyper-V
enlightenments documented, it may be unclear what is the recommended set to
achieve the best result. Add the corresponding section to the doc.

Windows with Hyper-V role enabled doesn't boot with 'hv-passthrough' when
no debugger is configured, this significantly limits the usefulness of the
feature as there's no support for subtracting Hyper-V features from CPU
flags at this moment (e.g. "-cpu host,hv-passthrough,-hv-syndbg" does not
work). While this is also theoretically fixable, 'hv-syndbg' is likely
very special and unneeded in the default set. Genuine Hyper-V doesn't seem
to enable it either.
Introduce 'skip_passthrough' flag to 'kvm_hyperv_properties' and use it as
one-off to skip 'hv-syndbg' when enabling features in 'hv-passthrough'
mode. Note, "-cpu host,hv-passthrough,hv-syndbg" can still be used if
needed.
As both 'hv-passthrough' and 'hv-syndbg' are debug features, the change
should not have any effect on production environments.

SEV-SNP support relies on a different set of properties/state than the
existing 'sev-guest' object. This patch introduces the 'sev-snp-guest'
object, which can be used to configure an SEV-SNP guest. For example,
a default-configured SEV-SNP guest with no additional information
passed in for use with attestation:

or a fully-specified SEV-SNP guest where all spec-defined binary
blobs are passed in as base64-encoded strings:

In some cases these blobs may be up to 4096 characters, but this is
generally well below the default limit for linux hosts where
command-line sizes are defined by the sysconf-configurable ARG_MAX
value, which defaults to 2097152 characters for Ubuntu hosts, for
example.

We can't just embed labels directly into files like qemu-options.hx which
are included from multiple top-level rST files, because Sphinx sees the
labels as duplicate: https://github.com/sphinx-doc/sphinx/issues/9707
So add an optional argument to the SRST directive which causes a label
of the form '.. _DOCNAME-HXFILE-LABEL:' to be emitted, where 'DOCNAME'
is the name of the top level rST file, 'HXFILE' is the filename of the
.hx file, and 'LABEL' is the text provided within the 'SRST()' directive.
Using the DOCNAME of the top-level rST document means that it is unique
even when the .hx file is included from two different documents, as is
the case for qemu-options.hx
Now where the Xen PV documentation refers to the documentation for the
-initrd command line option, it can emit a link directly to it as
'<system/invocation-qemu-options-initrd>'.

Add notes about console and network support, and how to launch PV guests.
Clean up the disk configuration examples now that that's simpler, and
remove the comment about IDE unplug on q35/AHCI now that it's fixed.
Update the -initrd option documentation to explain how to quote commas
in module command lines, and reference it when documenting PV guests.
Also update stale avocado test filename in MAINTAINERS.

KVM PV features don't seem to be documented anywhere, in particular, the
fact that some of the features are enabled by default and some are not can
only be figured out from the code.

Currently, cpu-models-x86.rst.inc is included in target-i386.rst directly.
To make the toctree more homogeneous when adding more documentation,
include it through a first-class .rst file.
Together with the previous changes to the man page skeletons, this also
frees "===" for the headings, so that cpu-models-x86.rst.inc need not
assume anything about the headings used by target-i386.rst.

Currently target-i386.rst includes the documentation of the 'pc'
machine model inline. Split it out into its own file, in a
similar way to target-i386.rst; this gives us a place to put
documentation of other i386 machine models, such as 'microvm'.
