iothread: Remove unused IOThreadClass / IOTHREAD_CLASS
Since commit be8d853766 ("iothread: add I/O thread object") we
never used IOThreadClass / IOTHREAD_CLASS() / IOTHREAD_GET_CLASS(),
remove these definitions.
