memory: make cpu_physical_memory_sync_dirty_bitmap() fully atomic
The fast path of cpu_physical_memory_sync_dirty_bitmap() directly
manipulates the dirty bitmap. Use atomic_xchg() to make the
test-and-clear atomic.
