spapr: Move typedef SpaprMachineState to spapr.h
Move the typedef from spapr_irq.h to spapr.h, and use "struct
SpaprMachineState" in the spapr_*.h headers (to avoid circular
header dependencies).
This will make future conversion to OBJECT_DECLARE* easier.
